Summit Family Dental offers great online tools through Patient Connect 365 to help you stay in control of your dental health. Securely access your complete treatment and account history 24/7, with your Patient Portal you can:
- Set up appointment reminders via email and/or SMS
- View upcoming appointments and appointment history
- Request your appointments online
- Make payments and view transaction history
- Access forms and documents
- Plus receive special discounts and promotions from our office
How to get started
When you schedule an appointment with us you will automatically receive an e-mail offering you the option to enroll, or click an enroll button below.
Set your password.
- Login and set your preferences.
Note: If you are going to be a patient at both of our offices, it is recommended that you use the same password for both accounts. This will ease access though the Patient Connect 365.
